I
have read and understand the terms and conditions as stated. ___________
_____I understand that the fee is $10 dollars / month for 100 megabytes of space, payable on the 1st day of the month. _________

understand that the fee is $25 dollars / month for 100 megabytes of space and
up to 20 email address, payable on the 1st day of the month.

understand that this is a 24-month commitment and that I will obey all the
terms and conditions set forth in the Term and Agreement statement. ________
Upon
signing this Contract I, ________________________agree to the statements above
and will adhere to the Terms and Agreements set forth in the Term and Agreement
statement.

understand that signing this agreement will give me 1 Username and password to
be listed below and that Meyer Electric Inc. will NOT release any username /
password information unless proper authentication has been received.___________
I
will pay the amounts as outlined above for 24 consecutive months due on the 1st
payable by the 15th and understand that if not paid by the 15th
that my connection and website will be terminated and only reconnected
after paying a $20 reconnection fee and any back dues.
